If your Mac Studio is failing to install macOS 26, you’re not alone.

Users with a M3 Ultra Mac Studio report installing macOS Tahoe via the System Settings app, wait for the update to complete, then end up booting back into macOS Sequoia 15.7.

The bug was first found by Apple Forum user Boschje79 suggests the bug is only affecting Mac Studios with a M3 Ultra chip.

Forum user FilipOfficial dug into his Mac Studios logs and said this:

The panic you’re seeing:
“ANEHWDevice::Failed to validate ANE register offset 0x5a8…”
…means the installer is loading the new Tahoe kernel/driver for the Apple Neural Engine, but the hardware check fails.

As of now, Apple is aware of the ongoing issue, and we hope they have a solution.

But our concern is how did Apple not catch this?
